If this is your first time, please have a look around! You may request appts, schedule changes, review labs and InBody forms, update demographics, and message your provider, all from your patient portal. 

Tasks: 
-Please check that all patient forms and documents have been signed and are up to date. These can be found under > Forms & Questionnaires. An exclamation next to this tab signifies that forms are due to be signed.
-Next, please review the tab: > My Account Details. Ensure demographics are correct, upload a photo, and opt in to texts & emails. 
-If insured, upload your insurance card and information. Though we do not accept insurance, we sometimes will need this information to process prior authorization requests for certain medications as well as lab processing if applicable. 

Optional: 
You may also update current prescription medications, supplements, allergies at any time.
