IMPORTANT PLEASE READ!!
REFILLS: Please request 2 weeks in advance. To expedite your request: If applicable: include the medication name, your current dose, tolerance, and the last four of the card to run if paid to the clinic. If you have a preferred card on file marked, this will be ran as default otherwise. This allows your provider at least 48-72 business hours to send it in and the compounders at least a week to prepare and ship, or if a commercial pharmacy then extra time for any PA requirements. Please do not request refills last minute with an expectation for a same day refill.
IF YOUR SHIPPING ADDRESS HAS CHANGED, YOU MUST NOTIFY US BEFORE SHIPPING! This needs to be changed in multiple locations on our end, please ensure we always have your most current address for all compound medication refills to avoid paying twice should your script end up delivered to an old address.
FORMS & CONSENTS:
-Please check that all patient forms and documents have been signed and are up to date. These can be found under > Forms & Questionnaires. An exclamation next to this tab signifies that forms are due to be signed.
-Next, please review the tab: > My Account Details. Ensure demographics are correct, upload a photo, and opt in to texts & emails.
-If insured, upload your insurance card and information. Though we do not accept insurance, we sometimes will need this information to process prior authorization requests for certain medications as well as lab processing if applicable and this will save you time later.
Optional:
You may also update current prescription medications, supplements, allergies at any time.
We kindly ask that cancellations are made with at least 24 business hours notice. Appointment no shows and cancellations that occur less than 24 business hours from your scheduled appointment time will incur an $85 fee. Monday appts must be rescheduled by end of business day on Fridays. We understand emergencies may happen. If you have a true emergent event contributing to a late cancellation, please cancel with us via phone at 317-708-0798 (even after hours), so that we can make attempts to fill your appt time with the goal to be able to waive your late cancel fee. No call / no shows, will automatically be charged $85 per our office policies. We appreciate your respect of our time and resources.
